At Synovum Care we believe it is very important that our customers have the opportunity to live as normal a life as possible with like-minded people, just as they would have done before requiring care and support services.  This lifestyle survey, which is a series of 22 questions that should take a few minutes to complete, will provide you – and us – with a comprehensive assessment of an individual’s Lifestyle Group.  This information will only ever be used to assist us in creating an environment where an individual’s lifestyle choices are respected – so that they can continue to live as “normal a life as possible”.

If you are completing the survey on behalf of a family member or friend, be aware that you need to answer the questions based on their beliefs not yours.
